Although PCM specializes in elder care we have many years of experience in a variety of care issues. We also work with a limited number of younger clients with difficult problems.

We Meet Your Needs

Emotional: The PCM staff is experienced in working with families under emotional stress. We understand the family complexities and know how to counsel in various situations.

Medical: Having worked within the medical community in Brevard County for many years, we can access the required systems and get the information required to complete a care plan.

Financial: As PCM develops a care plan, we ascertain what the cost
of care will be, often including assistance for others in the family. We are
always careful to use assets wisely.

Our Services

Private Case Management, Inc. works with individuals and/or families and accepts referrals from physicians, attorneys, community organizations, and agencies, as well as individuals. Your information will always remain confidential. We will make visits to private homes, nursing homes, assisted living facilities, or you may make an appointment to come in to our office to visit.

  • We evaluate and assess the situation and propose an effective and efficient personalized plan to maintain the individual in the least restrictive but most appropriate environment.

• Financial Management
  • We interpret and explain the maze of communications and correspondence that often overwhelms the elderly consumers. We manage their financial affairs, ensuring that payments are made in a timely manner. Emphasis placed on preserving and protecting the family assets.
