Private Case Management is a community outreach program that believes in showing honor to the elderly, developmentally and mentally disabled by providing various resources and services to assist them in having optimal care while maintaining their independence. PCM serves as a liaison between the client, court, family, physician, therapist, insurance company, hospital, and other important systems that support the welfare of the person. A Case Manager coordinates, manages and carefully tailors options in order to meet individual needs. The Case Manager is focused on balancing cost and quality so that resources are used appropriately. Since many systems that clients have to work with are fragmented and complex, the case manager must have a broad-base of knowledge including: medical, financial, social work, and more.

Catastrophic PCM is often involved in situations that affect many family members and require answers to a multitude of questions quickly. We provide services wherever the client is most comfortable.

Chronic PCM can assist with long range planning and allocation of resources, as well as planning respite for the caregiver to prevent burnout.

Complex PCM can respond quickly and efficiently to solve unusual problems in creative and innovative ways.
